Findings: The Alleged Victim (AV) is bed bound and relies on the facility staff for care. On or about December 19, 2023, AV asked a visitor to assist with getting him/her out of his/her brief. The visitor checked the brief, and it was soaked and the date/ time on the brief was 20 hours earlier. AV had sat in a soiled brief for over 20 hours. AV is care planned to have his/her brief changed 4 times per shift on day and swing and two times per shift on night shift. AV's brief was not changed for 20 hours, leaving AV at risk for harm and in unreasonable discomfort. The facilities failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
