Findings: According to the documentation, the facility failed to follow the care plan for Witness 1 (W1) to observe the resident when they are in common areas due to history of resident-to-resident altercations. The failure resulted in W1 pushing the Alleged Victim (AV) in their wheelchair and let go. This caused AV to crash into a table resulting in a skin tear to the AV’s hand which required stitches, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00295 $1125.00 fine assessed
