Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim’s (AV) care plan for their known fall risk. On or about December 04, 2023, the AP2 did not put on the AV’s nonslip socks as outlined in the care plan to prevent falls resulting in the AV falling and suffering an injury, which is a violation of resident rights is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
