Findings: According to the documentation, the facility failed to implement appropriate fall interventions for the Alleged Victim’s (AV) known fall history. On or about December 22, 2023, the AV attempted to self-transfer and was found on the floor in their apartment with their arms looped in the wheelchair causing pain and bruising, which is a violation of resident rights, is considered neglect of are and constitutes abuse.
