Findings: The Alleged Victim (AV) has an ostomy bag, which he/she can change independently mostly, however, does need some assistance and will ask staff to assist when necessary. AV's service plan does not specify what tasks or processes that staff need to assist AV with regarding his/her ostomy bag, even though several chart notes document AV's frustration with the bag. The facility's failure to provide assistance with AV's bag led to a loss of personal dignity for AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
