- According to the documentation, the facility failed to follow the care plan to provide incontinence care to the Alleged Victim (AV) timely. The failure resulted in the AV to not have their incontinence supplies changed timely causing skin breakdown and pressure sores, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
