Findings: The Alleged Victim (AV) is known to sleep on the edge of his/her bed. On or about December 30, 2023, AV was found on the floor in his/her room, with blankets wrapped around his/her legs and bleeding from his/her head. AV had fallen out of bed and hit his/her head on the night stand. AV was sent to the hospital for a laceration to the head. The facility failed to properly care plan and implement interventions to keep AV from sleeping on the edge of the bed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
