Findings: The Alleged Victim (AV) relies on the facility for care and has a recent history of falls. According to an investigation, on or about December 24, 2023, AV had an unwitnessed fall out of bed, resulting in a fractured hip. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
