Findings: The Alleged Victim (AV) relies on the facility for care, is a fall risk, and is care planned for staff to monitor AV to optimize safety. According to an investigation, on or about December 14, 2023, AV was observed to be wandering around the facility. Staff attempted to redirect AV, but AV wanted to be left alone. A short time later AV was found outside in the courtyard on their knees from an unwitnessed fall, resulting in AV hurting their finger. The facility failed to follow AV's care plan to monitor AV for safety, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
