Findings: Witness 1 (W1) had been becoming increasingly more aggressive towards staff and resident in the weeks leading up to the incident including physical assaults on other residents. On or about January 5, 2023, at breakfast time, W1 struck Alleged Victim (AV) in the back of the head two times as AV was sitting at a table for a meal. As a result of the incident, AV reported feeling fearful of W1. The facility failed to provide a safe environment putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00730 $500.00 fine assessed
