Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV was care planned for staff to make all arrangements for transportation and appointments. According to an investigation, AV's medical appointment was rescheduled due to lack of staff, resulting in continued unreasonable discomfort. The facility failed to provide appropriate services, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
