Findings: The Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) has a history of resident-to-resident altercations and is care planned to be redirected with full assistance from staff when wandering into other residents’ rooms. According to an investigation, on or about January 07, 2024, W1 wandered into AV's room and as AV was attempting to escort W1 out of the room, W1 struck AV on the right side of his/her face, resulting in a bruise. The facility failed to provide a safe environment, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
