Findings: The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of attempting to elope from prior facilities. According to an investigation, on or about December 8, 2023, AV eloped multiple times from the secured unit by disarming window alarm, exiting through a window and climbing a fence. AV's service planned interventions included frequent safety checks and replacing the window alarm, however the interventions were ineffective. The facility failed to properly plan care and prevent AV's elopement which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: RCFCP25-01360 $1500.00 fine assessed
