Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) care needs to prevent future falls. On or about October 20, 2023, the AV’s family had a care conference about not leaving the AV alone in their room in their wheelchair after dinner as an intervention for the AV’s known history of anxiety and self-transfers. On or about October 20, 2023, the AV was left alone in his/her bedroom in their wheelchair after dinner at approximately 5:30 pm. The AV was found at approximately 8:00 pm on the floor in their room. The facility staff assisted the AV up off the floor and into a chair, then to their bed before calling 911. EMS took the AV to the hospital where they were diagnosed with a fractured hip which required surgery, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
