Findings: The Alleged Victim (AV) had a history of falls. The AV experienced falls in November and December of 2023, some with and without injury. The facility implemented some interventions after AV's November 28, 2023 fall, however, AV suffered 3 more falls, with the fall on December 27, 2023 resulting in AV suffering a skin tear and increased back pain. On or about December 28, 2023, AV was transported to the hospital and diagnosed with a vertebral fracture and had to be sent to a skilled nursing facility for rehabilitation. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
