Findings: The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about January 9, 2024, AV did not receive monitoring and wound care for pressure wounds to the heels. Over a period of at least 21 days, AV's wounds were not treated, resulting in AV's wounds worsening, requiring a hospital visit and unreasonable discomfort. The wounds required intervention and routine treatment. The facility failed to provide oversight and monitoring of the wounds and failed to provide appropriate skin care, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
