Findings: The Alleged Victim (AV) relies on the facility for their care and has a history of falls. AV is care planned for a two-person transfer and is known not to push call pendant to have staff assist with transfers. According to an investigation, between November 3, 2023, and December 16, 2023, AV experienced multiple falls while attempting to self-transfer, resulting in repeated unreasonable discomfort. The facility failed to provide appropriate services, to include care planning and/or implementation of interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse., which is a violation of resident rights, is neglect of care which constitutes abuse.
