Findings: On or about January 17, 2024, Alleged Victim (AV) suffered two falls back-to-back in which AV suffered injuries on the second fall and was sent to the hospital for treatment. Based on facility documentation and interviews, the facility failed to monitor and take the necessary safety precautions after AV's first fall which resulted in AV falling again shortly after the first fall. The facility's failure caused AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP24-00188 $1125.00 fine assessed
