Findings: Alleged Victim (AV) relies on the facility to ensure AV’s basic care needs are met. AV is dependent on staff to check (Metabolic Marker) levels and ensure the levels are at baseline. AV had four (4) separate falls due to him/her having low (Metabolic Markers) on or about September 4th, 16th, 23rd, and October 4th, 2023. As a result of some of those falls, AV sustained a skin laceration to the elbow. AV also complained of hip pain from the fall that occurred on September 23, 2023. The facility failed to follow the care plan for AV when it comes to the snack plan or testing for low (Metabolic Markers) which resulted in falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
