Findings: The Alleged Victim (AV) is care planned for staff to watch AV’s apartment to make sure no other residents enter as he/she is known to be territorial of his/her things and the apartment. Witness 1 (W1) has a history of resident-to-resident altercations. According to the investigation, on or about December 20, 2023, AV and W1 were in a resident-to-resident altercation when W1 entered AV’s apartment. AV and W1 fought over AV’s cane and W1 hit AV on the head and AV hurt his/her fingers. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
