Findings: On or about December 31, 2023, at approximately 3:49 AM, the Alleged Victim (AV) had rolled out of bed and complained that his/her wrist hurt worse, as AV had previously complained that his/her wrist hurt. At approximately 11:35 AM, staff discovered that the Alleged Victim's (AV) left arm and wrist was very swollen and looked as if it were broken. Later that day at 3:45 PM, AV was placed on alert for increased pain in the wrist. AV's family member showed up the next day and determined to take AV to the hospital where he/she was diagnosed with a fractured wrist. The facility's failure to ensure timely medical treatment placed AV in unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
