Findings: The facility failed to implement interventions and appropriately monitor Witness 1 (W1) according to his/her known behavior and prior altercations. On or about December 24, 2023, W1 walked up to Alleged Victim (AV) and hit AV in the back of the head with three (3) briefs in his/her hand. Based on documentation and interviews, W1 targets AV and staff tries to keep W1 and AV separate. At time of incident, staff were in the area and did not redirect W1 from AV. The facility's failure to redirect W1, caused unreasonable discomfort to the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
