Findings: On or about January 25, 2024, Alleged Perpetrator 2 (AP2) used physical force against Alleged Victim (AV) while working as AV's caregiver as an employee of the facility. Based on facility documentation and interviews, AP2 was assisting AV with toileting when AV became combative. AP2 failed to follow AV's care plan regarding how to approach AV when AV's is exhibiting behaviors. Instead of requesting assistance from other caregivers, AP2 allowed the situation to escalate by not following AV's care plan resulting in AP2 pushing AV which caused AV to lose his/her balance resulting in AV falling and hitting his/her on the closet door. AP2's actions is considered physical abuse which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
