Findings: On or about January 28, 2024, Alleged Victim (AV) eloped from the memory care unit. At time of elopement, the magnetized locking mechanism on the memory care unit doors were inoperable due to issues with the fire system after a pipe burst on the 3rd floor. According to documentation, there was no injury to AV at time of incident. The facility failed to take preventive measures to prevent AV from eloping and provide a safe environment, which is a violation of Oregon Administrative Rules.
