Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) used failed to ensure the Alleged Victim (AV) was safe. On or about January 30, 2024, when AP2 used the keypad to enter the locked memory care facility, the AV was at the door and the AP2 let AV out creating a risk of harm to the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to prevent elopement which is a violation of Oregon Administrative Rules.
