Findings: The facility failed to provide appropriate services according to the Alleged Victim’s (AV) care needs resulting in AV sustaining skin discoloration to AV's body, during the month of January 2024, in multiple locations. Based on facility documentation and interviews, AV is bedbound and requires staff assistance with all Activities of Daily Living (ADLs). AV had skin discolorations documented on January 23, 2024, and on January 27, 2024. The facility failed to ensure staff were properly trained on providing care and failed to ensure AV's bed pad used for repositioning AV was appropriately sized. The facility's failure caused AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
