Findings: On or about January 22, 2024, Alleged Victim (AV) was observed to have two oval red marks on his/her left buttock and a bruise on his/her left forearm. AV is a full assist with all Activities of Daily Living and completely relies on staff to meet his/her needs. At time of incident, the facility was short-staffed, when AV's bruising and redness occurred. Based on facility documentation and interviews, it was determined that the facility failed to properly train staff and implement person centered interventions to reduce the risk of AV suffering position related injuries, which resulted in AV suffering redness to his/her left buttock and bruising to him/her left forearm. The facility's failure to document and implement person-centered interventions, resulted in AV suffering position related bruising and redness which caused AV unreasonable discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse. An unknown Alleged Perpetrator 2 (AP2) allegedly neglected AV. An investigation determined no abuse occurred by AP2.
