Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about January 30, 2024, staff noted AV having increased confusion and was anxious all day. After lunch, AV was walking back and forth in the hallway when around 2:05 PM, AV was heard falling. From after lunch until AV fell, staff did not offer any interventions in the care plan to calm AV. As a result of the fall, AV was found lying on AV’s back on the floor screaming out in pain while lying on the floor in the hallway with his/her right shoulder bulging out. The facility failed to follow the care plan around AV’s known anxiety, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP25-00337 $500.00 fine assessed
