Findings: Witness #2 (W2) has a history of being aggressive and having altercations with other residents. On or about January 4, 2024, W2 was seen aggressively pulling on the Alleged Victim's (AV) arm, which resulted in a scratch on AV's arm. The facility failed to monitor, intervene and put interventions in place to keep AV safe. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
