Findings: Both the Alleged Victim (AV) and Witness 4 (W4) rely on the facility for care. AV and W4 both have a history of aggressive behaviors resulting in physical altercations and the AV has a history of wandering into other resident rooms. On or about February 4, 2024, W4 was observed dragging AV out of another resident’s room. AV had a bloody nose. The facility failed to implement effective interventions to address both AVs and W4s aggressive behaviors, and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
