Findings: According to the documentation, the facility failed to implement appropriate interventions for the Alleged Victim’s (AV) known fall history. On or about February 04, 2024, the AV experienced a fall, and the next day was found to have a large bruise on their back. The interventions put into place by the facility were not effective in preventing the AV from falling resulting in a large bruise and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00333 $375.00 fine assessed
