Findings: According to the documentation, the facility failed to follow the care plan by transferring the Alleged Victim (AV) to bed. On or about January 22, 2024, the facility staff did not transfer the AV to their bed and left them in their wheelchair all night causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
