Findings: The Alleged Victim (AV) was care planned to have his/her weight checked weekly. The facility only weighed AV one time during December 2023. AV lost 20 pounds between November 2023 and January 2024. Staff noticed that AV did not look well January 31, 2024. Facility staff contacted AV's physician on February 2, 2024 to ask to send AV to the hospital. AV was transferred to the hospital and was diagnosed with severe malnutrition. The facility's failure to have oversight and respond to AV's change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00443 $500.00 fine assessed
