Findings: Alleged Victim (AV) is bedbound and per his/her service plan, requires eight (8) checks per shift. On or about January 22nd, 23rd, 24th and 25th, 2024, during the night shift, staff failed to check AV as required per the service plan. On or about February 5th, 6th, 7th, and 8th, 2024, during the night shift, staff failed to check AV as required per the service plan. As a result, AV suffered discomfort and skin breakdown. The facility failed to follow the service plan, resulting in skin breakdown to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00964 $250.00 fine assessed
