Findings: Witness #1 (W1) has an extensive history of resident to resident altercations. On or about February 3, 2024, the Alleged Victim (AV) was sitting on a bench in the hallway when W1 approached AV and got upset over AV's presence. W1 began to yell and threw a cup of water on AV. Staff intervened and directed AV to a quieter area. The facility failed to properly care plan regarding W1's behaviors, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01213 $375.00 fine assessed
