Findings: On or about February 20, 2024, Alleged Victim (AV) fell onto the floor of his/her room while attempting to sit on a chair. At time of incident, AV's room was dark, no lights were on. Based on facility documentation and interviews, AV suffers from blurred and distorted vision and is care planned to have a soft, non-glaring light on at night. AV's care plan was not being followed as all lights in AV's room were off. The facility's failure to follow AV's care plan, resulted in AV falling, suffering unreasonable discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse.
