Findings: The Alleged Victim (AV) relies on the facility for his or her safety and is a known exit seeking resident with an elopement risk. According to an investigation, on or about February 23, 2024, the AV eloped from the facility through an unsecured door. AV was found in the kitchen near hazardous equipment and sharp knives placing AV at risk for harm. AV was escorted by kitchen staff back to the locked memory care unit. Memory Care staff did not know AV had eloped. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
