Findings: According to the documentation, the facility failed to properly care plan appropriate interventions for Witness 2’s (W2) known aggressive behavior. On or about February 11, 2024, W2 hit the Alleged Victim (AV) in an altercation the facility did not implement any interventions to prevent future altercations. W2 had another resident-to-resident altercation with AV on or about February 26, 2024, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
