Findings: The Alleged Victim (AV) was a known fall risk with a history of falls. According to an investigation, AV experienced multiple unwitnessed falls between February 2, 2024, and February 28, 2024, resulting in pain, skin tears, and sutures. The facility failed to provide appropriate services according to the AV's needs relating to the number of falls with lack of appropriate interventions. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
