Findings: On or about February 29, 2024, the Alleged Victim (AV) suffered a fall in his/her room, and had suffered a previous fall on February 23, 2024. The facility call system was not working on February 29, 2024, and it is unknown when AV was last checked on or how long AV was on the floor calling for help. AV was not injured in this fall, but being on the floor for an unknown amount of time caused AV unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
