Findings: The Alleged Victim (AV) relies on the facility for care and has a history of falls. According to an investigation, between March 2, 2024, and March 5, 2024, AV fell approximately three times, with at least one fall resulting in AV being sent to the hospital. The facility failed to put effective, person-centered interventions in place after repeated falls, which is a violation of resident rights is neglect of care and constitutes abuse.
