Findings: Witness #1 (W1) has a history of resident to resident altercations and is to have eyes on him/her when in the common area of the facility. On or about March 9, 2024, W1 and the Alleged Victim (AV) engaged in an altercation in the common area of the facility where W1 grabbed AV, leaving marks on AV's neck and arms. Staff are to watch and intervene if they observe W1's behaviors change. The facility failed to have enough staff available to provide eyes on care for W1, allowing the altercation to occur. The facility's failure to provide enough staff placed AV at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
