Findings: The Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim’s (AV) or Witness 1’s (w1) care plans related to behaviors and agitation. On or about March 9, 2024, W1 was exhibiting signs of agitation toward AV for over an hour and was unable to be redirected. AV became upset and screaming for assistance. W1 and AV were not separated, causing a physical altercation resulting in AV being struck in the face and receiving a bloody nose. AP2’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility’s failure to provide a safe environment is a violation of Oregon Administrative Rules.
