Findings: On or about March 11, 2024, the Alleged Victim (AV), a resident of the memory care unit, was able to elope by bypassing two secured doors and exiting the facility without staff knowledge. The facility failed to adequately monitor AV’s known exit-seeking behaviors, despite documented risk factors. This lapse in supervision resulted in the resident being placed at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP25-01013 $188.00 fine assessed
