Findings: The Alleged Victim (AV) is a known fall risk, and primarily falls while in his/her room and is on frequent checks with staff. On or about March 10, 2024, AV fell while transferring from his/her wheelchair to his/her bed. AV was sent to the emergency room where he/she was diagnosed with four fractured ribs. The facility's failure to ensure sufficient interventions were in place to ensure AV's safety from falling is a violation of resident rights, is considered neglect of care and constitutes abuse.
