Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to protect the Alleged Victim (AV) from verbal abuse. On or about March 12, 2024, the AP2 and AV were texting about the AV’s medication. During this texting conversation, the AP2 responded with an inappropriate text using emotionally harmful language, which is a violation of resident rights and is considered verbal abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
