Findings: On or about March 8, 2024, the Alleged Victim (AV) was found on the floor next to his/her bed. AV had dried feces on his/her hands and clothing. It is unknown how long AV had been on the floor, as AV is care planned for 2 hour checks, however, no documentation of the checks exists. AV has behaviors of digging in his/her feces if he/she is constipated, which has been going on for a couple of months. The facility did not complete a change of condition for AV, nor update his/her care plan for this new behavior when it began. The facility's failure to provide oversight and monitor AV's change of condition placed AV at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
