Findings: The facility failed to provide a safe environment. According to an investigation, on or about January 29, 2024, Witness 1 (W1) was agitated and followed a staff member into the hallway, where he/she punched the Alleged Victim (AV) in the back and in the head, resulting in unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
