Findings: The Alleged Victim (AV) relies on the facility for care and has a history of falls. According to an investigation, on or about March 18, 2024, AV had an unwitnessed fall, resulting in bruising to AVs face, orbital socket, and hand, and a bleeding head wound. The AV was taken to the hospital for treatment that included closing the head wound with glue. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
