Findings: Witness #1 (W1) is known to have behaviors and engage in altercations. On or about February 18, 2024 and February 25, 2024, the Alleged Victim (AV) was physically attacked by W1, causing injury to AV. AV was attacked again on March 11, 2024 without injury. The facility's failure to keep AV safe from attacks by W1 is a violation of resident rights, is considered neglect of care and constitutes abuse.
